Golfers: Your Back Pain Probably Isn't a Back Problem
If you golf and deal with low back pain, here's a pattern I see constantly in the clinic: it's almost never the swing itself causing the problem. It's what the hips can't do before the swing even starts.
Two restrictions show up over and over:
1. Lack of hip extension (lead leg) During follow-through, your lead hip needs to fully extend so the pelvis can rotate through the shot. When that extension is limited, the low back extends instead - every single swing, that load goes somewhere it shouldn't.
2. Lack of hip internal rotation (trail leg) During the backswing, your trail hip needs internal rotation to load properly. When it's restricted, the spine rotates to make up the difference. Over 80-100 swings a round, that repetitive rotational stress adds up fast.
The common thread: when the hip can't move the way the swing demands, the low back moves for it. That's not a strength problem or a "engage your core more" problem - it's a mobility problem, and it's specific to two exact directions of hip motion.
What this means for your game:
The fix isn't more crunches or planks. It's restoring the specific hip mobility that's actually missing - which is different for every golfer, and different depending on which side is restricted.
This is exactly what we assess in the Golf Movement Health Screen: hip extension, internal rotation, and swing-specific loading patterns, tested before pain shows up rather than after.
